Here you can find the total premium price and any specific taxes and levies applicable in your country of residence. You'll also have a breakdown of how much of the premium will be paid by your employer and how much will be paid by you (deducted from your salary - for which you'll have to authorise on the bottom agreement checkbox). In some cases, you might select a health insurance plan that is equal to or cheaper than the set contribution defined by your employer. In that case, you won't have to pay anything.
The date of coverage and the number of months included in the quote might not be for 12 months. This is because the plan's renewal date is defined by the moment your employer contracted this health insurance policy from the provider, and not when you enrolled. For instance, if your employer originally set up the insurance in July, and you're being added to the benefits package in December, you will be part of the employer package for 7 out of 12 months.
